Student-Athlete Checklist

Welcome to the Mighty Oaks Athletics program! Below you will find information that must be completed prior to your competing with the team. This information allows us to gather all the information we need to certify you as an eligible USCAA student-athlete. It is your responsibility to ensure that all information is accurate and complete. Any omissions, deletions, or falsifications will result in disciplinary action by ESF and the USCAA.

1.) Follow all fo the steps on the Admissions Accepted Students Checklist

2.) Student-Athlete Physical Form. NOTE: Please download all forms. For assistance, please contact Sofia Hvozda - Administrative Assistant  shvozda@esf.edu (315)470-4863 or your individual coaches. Physicals must be signed by the doctor.

3.) Please download and read the Student-Athlete handbook. There is important information in this handbook including USCAA and ESF athletics eligibility requirements.

4.) If you are a new or transfer student-athlete, please feel out the Student-Athlete Intake Bio Form
